Step 1: You can book online or call our office to reserve your time!
Step 2: Complete digital forms: You’ll receive a secure link via email or text to fill out intake and consent
forms.
Step 3: Confirmation: Our office will confirm your appointment 24–48 hours before your scheduled time.
Step 4: Arrival: Check in with the front desk upon arrival and have your ID and payment method ready.
Please bring the following items to your first visit:
● Valid photo ID (driver’s license or government-issued ID)
● Insurance card (if applicable)
● Referral form or authorization (if required by insurance)
● List of current medications and relevant medical records or test results (Any recent lab results or imaging related to your care.)
● Method of payment for co-pays, deductibles, or out-of-pocket services
Please complete all digital intake paperwork before arrival. Any cancellations or reschedules must be made at least 24 hours in advance to avoid fees.
We are located at 1425 Georgia Avenue, Suite 201C, Macon, Georgia 31201. Convenient parking is available adjacent to the building. Enter through the side entrance and take the elevator to the second floor to reach Suite 201C.
● Front Desk Staff: Assists with check-in, verifies your information, and answers questions.
● Medical Assistant (MA): Reviews your health history, medications, and prepares you for your visit.
● Provider/Physician: Discusses your goals, performs your evaluation, and recommends a personalized treatment plan
Please arrive 10–15 minutes early for your appointment to allow time for parking and check-in. If you arrive more than 10 minutes past your scheduled time, your appointment may need to be rescheduled.
If your visit involves aesthetic or skin procedures, please arrive with a clean face (no makeup or skincare products). Certain procedures may have specific pre-visit instructions that our staff will provide ahead of time.
Disclose all medications, supplements, and allergies prior to your visit.
